(1) Vendor needs to provide mobile integrated health services to the government authority located in Iowa.– The purpose of developing Mobile Integrated Health projects is to initiate or expand treat in place care, telehealth–enabled clinical assessments, chronic disease management, post discharge follow–up, prenatal and postpartum home–based support, medication reconciliation, and other community–based clinical interventions.– Two options for advancing mobile integrated health projects.– Initiate a new Mobile Integrated Health Program. – Expand an existing Mobile Integrated Health Program to cover additional rural counties.  (2) The contract period will be for five years.(3) All the questions must be submitted no later than July 27, 2026.
