(1) Vendor needs to provide day treatment intensive and day rehabilitation services to the government authority located in Yreka, CA. – Provide specialty mental health services for eligible medical beneficiaries residing in agency.– Provide Day Treatment Intensive (DTI), Day Rehabilitation (DTR), or both services for eligible medical beneficiaries who require structured behavioral health treatment in a community–based setting.• Deliver services that are recovery–oriented, trauma–informed, culturally responsive, person–centered, and clinically appropriate.• Provide services in accordance with applicable agency medical specialty mental health services requirements.• Ensure services are provided in the least restrictive setting appropriate to each client's clinical needs.• Services shall incorporate family, natural supports, and community partnerships whenever clinically appropriate.• Complete timely intake, assessment, and admission processes.• Develop individualized treatment plans.• Provide individual, group, family, and rehabilitative services, as appropriate.• Coordinate care with agency behavioural health, primary care providers, schools, hospitals, social service agencies, housing, employment, education, transportation and other community partners, when appropriate.• Develop discharge and transition plans to support continuity of care.(2) All the questions must be submitted no later than August 25, 2026.
