Public Safety Behavioral Health Assistance Program (BHAP)

The City of Surprise, Arizona (the "City") is seeking Statement of Qualifications (SOQs) from qualified respondents to provide a comprehensive Behavioral Health Assistance Program (BHAP) specifically designed for the members of the public safety departments. It is the intent of the City to award a multi-term contract to one (1) or more qualified respondent(s) who are deemed the most responsive and responsible based on their demonstrated experience in providing BHAP services based on the State of Arizona requirements, including but not limited to respondent(s) qualifications, degrees, certifications, scope of work and the evaluation criteria specified herein. The award will be limited to the least number of respondent(s) that the City determines is necessary to meet the needs of the public safety agencies for an initial two (2) year term contract with an option to renew annually up to an additional three (3) one-year terms.
