Vendor needs to provide transition service to the government authority located in Boston, MA. – Agency seeks to identify and qualify a list of organizations interested in and available to provide Transition to Adulthood Program (TAP) for students with disabilities helping them prepare for adulthood and achieve greater independence.– The Transition to Adulthood Program (TAP) provides individualized independent living skills training and support services to transition–aged students with disabilities preparing for adulthood. Services should take into consideration student strengths, interests, and independent living needs; development of individualized goals and objectives; and ongoing instruction and support designed to increase independence, self–determination, and successful transition outcomes.
