(1) Vendor needs to provide independent physician incubator service to the government authority located in Hawaii. • Provide financial and technical assistance to support innovative, evidence–based strategies that address the unique health challenges facing rural populations. • Program activities may include chronic disease prevention and management, behavioral health services, workforce development, technology enabled healthcare solutions, data–driven public health planning, healthcare system transformation, and the promotion of healthy lifestyles, including increased physical activity.• Develop, coordinate and implement a statewide strategic plan to strengthen independent physician practice and infrastructure needed to address the state’s primary and specialty care shortage.
