(1) Vendor needs to provide Warm Hand Off (WHO) support and care coordination service to the government authority located in Erie, PA. – Provide care navigation and coordination model, including integration of staff and, where proposed, other peer recovery support staff.– Ability to facilitate and coordinate timely access to Level of Care (LOC) assessments, which may be conducted, participating treatment providers, or other designated assessment entities utilizing in–person and secure virtual assessment methods, as appropriate.– Ability to coordinate treatment admission logistics, transportation needs, and follow–up communication with assessment providers, treatment providers, and recovery support organizations to support successful door to door linkage to care.– Ability to obtain and maintain all hospital credentialing, onboarding, security clearance, training, confidentiality, and electronic access requirements necessary to perform services within hospital settings.– Integrated into hospital workflows, including timely bedside response, participation in discharge planning, coordination with medical, social work, and case management staff, involvement in interdisciplinary communication processes, and utilization of hospital referral, notification, and care coordination systems.(2) All the questions must be submitted no later than July 17, 2026.
