Description
The County of Napa (hereinafter referred to as “the County”) is inviting proposals from qualified firms (hereafter referred to as "Proposer", "Proposers", “Bidder”, “Bidders”, “Vendor” or “Vendors”) to administer health care services with the Napa County Jail and Juvenile Detention Facility. The embedded health services shall be inclusive of medical, dental, optometry, mental health, substance use, medication management, and pre-release services required by the California Department of Health Care Services (DHCS) for the CalAIM Justice Involved Reentry program. The County seeks qualified Bidders with demonstrated experience providing a comprehensive and cost-effective program in correctional setting for average daily populations of at least 1,000. Bidders are responsible for all requirements and responsibilities outlined in the Scope of Work and specifications. Bidders must provide a Cost Proposal breakdown for all areas as listed in this RFP. It is the intent of this RFP to locate qualified Bidders who are interested and able to provide and coordinate comprehensive health care services (including preventive, behavioral health, optometry, and dental), pre-release services, medical billing (optional), and associated systems of care for incarcerated adults in the Napa County Department of Corrections (NCDC) and for incarcerated juveniles in the Napa County Probation Department (Probation). The selected Vendor shall work cooperatively with NCDC and Probation, the Napa County Health and Human Services Agency (HHSA), local hospitals and any behavioral health providers. The provision of embedded health services by the Vendor shall include comprehensive medical care including administering and prescribing medicine, women’s health, dental care, behavioral health care, clinic time and other services as more specifically identified in this RFP. The County intends to award a three (3) year contract (with two (2) year annual options to renew) to the Vendor selected as most qualified in providing the medical health care services that meet the needs of the County’s incarcerated adults and juveniles. The primary goal of the County in requesting these services is to purchase quality health care services in an efficient and effective manner at a cost reasonable to the County. Each respondent to the RFP shall be evaluated as to its achievements and compliance with the following stated objectives: To deliver high quality comprehensive health care services (including preventive, medical, dental, optometry, mental health and substance use treatment) that can be audited against established standards. To operate a comprehensive health care program (including preventive, medical, dental, optometry, mental health and substance use treatment) in a cost-efficient manner with full reporting and accountability to the Napa County Department of Corrections and Probation Department or their designees. To operate the comprehensive quality health care program (including preventive, medical, dental, optometry, mental health and substance use treatment) at full staffing, full shift coverage, and use only licensed, certified, and professionally trained personnel. To implement a written health care plan that is inclusive of preventive, medical, dental, optometry, mental health and substance use services with clear objectives, policies, and procedures for annual evaluation of compliance. To maintain an open and cooperative relationship with the administration and staff of the Napa County Department of Corrections and Probation Department. To provide a comprehensive program for continuing in custody staff education. To maintain complete and accurate records of care and to collect and analyze health statistics on a regular periodic basis. To operate the health care program in a humane manner with respect to the individual’s right to basic health care services, including preventive, medical, dental, optometry, mental health and substance use services. To provide for a fair and objective proposal that will result in a mutually satisfactory contract between the successful Vendor and Napa County. Objectives of the RFP The scope of the RFP matches the current operating model for health services in the adult and juvenile detention facilities and is referred to as a “mixed model”. The RFP is focused on administering physical health services, pharmacy, medication assisted treatment, dental, CalAIM pre-release, and other care coordination services in the adult and juvenile detention facilities and maximizing Medi-Cal revenue within those service lines. Under the mixed model, the administration of pharmaceuticals for individuals detained in the facility, or being provisioned with medications at time of release, would be the sole responsibility of the Vendor . In addition to the Bidder’s primary role, Napa County Health and Human Services Agency (HHSA) provides, pursuant to an ongoing request by the Corrections and Probation Departments, licensed behavioral health staff (8.0 FTEs) who would continue to administer specialty mental health services in the County Jail, Juvenile Hall, and Skyline Academy. The County’s HHSA also administers the behavioral health linkages for adults and juveniles for post-release services. Optional services for Bidders include the Medi-Cal billing and revenue cycle management for all services rendered in the adult and juvenile detention facilities, including the services rendered by County HHSA behavioral health staff. Napa County intends to release a separate procurement process for the Medi-Cal billing services for the CalAIM forensic services. The Vendor would be required to integrate their staff with the County HHSA behavioral health staff in the facility for purposes of care coordination and continuity of care, access to the Vendor’s electronic health record system, as well the sharing of clinical documentation created by HHSA staff that HHSA may require be contained within their electronic health record system for continuity of care outside the Jail, Juvenile Hall, and Skyline Academy. The Vendor and County would collaborate to deliver a unified set of health services for individuals in the detention facilities, and for pre-release and re-entry planning, and would be responsible for aligning services to the greatest extent possible with Medi-Cal billing to maximize revenue for all allowable services. The Bidder has the option to exclude Medi-Cal billing and revenue cycle management services from the proposal. The Bidder shall be expected to forecast and project revenue with a sound basis for such projections accompanied by an implementation plan for Medi-Cal billing from initial launch through to a timeline for reaching maximum capacity. The County would be responsible to administer the oversights and internal controls related to contract management, regulatory compliance & auditing, program integrity, and revenue cycles. Vendors shall use this section to describe how they will meet the general obligations, standards, and objectives of the comprehensive health care program. Provide information regarding the background, expertise, qualifications, and philosophy of Vendor’s organization. Vendors are encouraged to demonstrate how best practices and lessons learned are integrated into a service model that promotes safety in the facility and the community. Vendor shall fulfill the following objectives. Submit a pricing proposal for the administration of physical health, mental health, substance use, and all other services in the County Jail, Skyline Academy, and Juvenile Hall. Ensure a collaborative working relationship with HHSA to administer the specialty mental health services in the County Jail, Juvenile Hall, and Skyline Academy. In addition to the administration of specialty mental health services, HHSA administers the behavioral health linkages for adults and juveniles in the detention facilities that are screened or assessed with severe mental illness. The Vendor shall coordinate with County staff and allow access to the electronic health record system and integrate the County staff into the care management teams in the adult and juvenile detention facilities. Ensure the health care program is run in a professional, efficient, and cost-effective manner with full reporting and accountability to County. Ensure that the health care services operate at full staffing, using only licensed staff who are working within their scope of licensure. Provide a comprehensive educational program for professional and sworn staff and to include quarterly onsite hands-on training. Collect, analyze, and provide health statistics to County on a weekly, monthly, quarterly, and annual basis as determined NCDC and Probation. The statistics should include at a minimum: Number of intake screenings performed and number of incarcerated persons who did not receive intake screening prior to housing. Number of health needs requests submitted monthly, number triaged the same day as collection, number of emergent conditions, number of emergent conditions seen immediately, number of urgent conditions, number of urgent conditions seen the same day the form is triaged, number of who received visits with providers within 14 calendar days of receipt of the form. Number of incarcerated persons indicating a clinical symptom in their health care request that were not seen by the Vendor’s licensed staff. The percent of incarcerated persons who did not receive their first dose of essential medication as determined by the medical provider within 24 hours of prescription. The percentage of incarcerated persons with medication lapses due to untimely renewals. The number of incarcerated persons verified as already on psychotropic medications at booking who do not receive their medications within 48 hours of a physician or psychiatric prescription order during intake (or, if a physician or psychiatrist does not order such medications during intake, who were not seen by the physician or psychiatrist within 24 hours of booking). The number of incarcerated persons who claim to be taking psychotropic medications at the time of booking for whom staff did not take reasonable steps to verify the prescription. The length of time from initial clinician referral to completion of specialty appointment by service. The number of incarcerated persons with chronic illnesses who did not receive their medications as determined by the medical provider within a day of the physician or psychiatrist’s medication order. The number of incarcerated persons with chronic illnesses who did not receive a history and physical examination by a registered nurse, physician’s assistant, nurse practitioner or physician within two (2) weeks of intake. Grievances over health care complaints and institutional responses. Court orders for health care and institutional responses. Attain and maintain NCCHC accreditation within negotiated timelines. Perform audit services and staffing through a continuous quality improvement program. Provide corporate nursing leadership to Vendor’s on-site Health Service Administrator (HSA) which will include bi-weekly onsite visits. Provide and maintain telemedicine services including computer equipment and all examining peripherals. Provide pharmacy management to include but not limited to quarterly onsite inspections, inventory management, cost containment measures, and reporting on drug costs, formulary changes, and other actions that can be taken to improve the quality and cost-efficiency of pharmaceutical care. Ensure the processing of Medi-Cal County Incarcerated Person Program (MCIP) applications of all hospitalized incarcerated persons admitted to an acute care setting more than 24 hours. To provide billing information to the hospital in order that MCIP can bill directly to the carrier. Operate the health care program with health care policies and procedures mutually agreed upon between NCDC and Probation. All policies and procedures, standing orders, practices and protocols must comply with NCDC and Probation policy and procedures and must be approved by the County prior to implementation. Implement a utilization review program. Implement a claims adjudication process. Ensure that Federal, State, and County requirements are met. Vendor shall refer to Exhibit C – Definitions.
