(1) Vendor needs to provide per diem nursing and care management services to the government authority located in Belvidere, NJ. – Per diem nurse to complete clinical eligibility screens and assessments for individuals seeking Medicaid benefits such as agency and other state funded home and communitybased program.• Per diem nursing staff:– Perform clinical eligibility screening and assessments nursing staff, which may require home visitscomplete documentation using the state based systems and follow through with clinical eligibilitydocumentation.• Per diem ADRC/PASP care manager(s):– Care manager shall attend annually a minimum of at least one continuing education training course or seminar related to older adults or persons with disabilities.
