(1) Vendor needs to provide frequent user system engagement program services to the government authority located in Bloomington, IL. – Maintain the core elements of the Frequent User System Engagement (FUSE) model and provide or coordinate the services and supports necessary to meet the complex and evolving needs of program participants. • Program operations and participant services:– Operate the FUSE Program using a person–centered, community–based approach that provides intensive engagement and individualized support to participants with complex behavioural health and social service needs.– Provide or coordinate services that support behavioural health, healthcare access, housing stability, connection to community resources, and progress toward individualized treatment or service plan goals.• Staffing and transition planning:– Develop a transition plan with agency that identifies key activities, responsible parties, milestones and an anticipated date for assuming program operations.• Community partnerships and service coordination: Maintain and strengthen relationships with behavioural health providers, healthcare systems, housing providers, criminal justice and law enforcement partners, crisis services, shelters, and other partners essential to the FUSE model.• Program administration and performance monitoring: – Establish procedures for program oversight, documentation, recordkeeping, confidentiality and compliance with applicable requirements.– Track relevant program data and outcomes, including participant engagement, services provided, housing stability, and connections to community resources. • Budget and long–term sustainability.(2) All the questions must be submitted no later than November 9, 2026.
