(1) Vendor needs to provide intensive case management services to the government authority located in Elmira, NY.– Shall ensure all services are delivered using a family or person–centered approach, strength–based practice, trauma–informed care, and innovative approaches. Services must recognize participant resilience, emphasize safety and empowerment, and use flexible, creative strategies to reduce barriers and improve outcomes,– Shall carry a defined caseload of referred households and conduct comprehensive, holistic assessments identifying strengths, needs, and opportunities to reduce reliance on public assistance. workers will co–develop individualized service plans focused on housing stability, income growth, and long–term economic independence. – Shall implement strategies to prevent homelessness or emergency placement, assist with housing searches and applications, support budgeting and landlord engagement, and provide assistance with move–in logistics and tenancy education,– shall provide post–placement support to reinforce stability and conduct follow–up monthly to monitor housing and income outcomes. Innovative follow–up strategies may be used to prevent returns to crisis.(2) All question must be submitted no later than July 24, 2026.
