HEALTH-10831 - USA (Wayne, New York) - Older Adults Home Personal Care Services - Deadline October 30,2026

(1) Vendor needs to provide older adults home personal care services to the government authority located in Wayne, NY. – Provide personal care services and housekeeping and chore services to county residents assessed and deemed eligible by agency staff.– These residents will be 60 years of age or older and be functionally impaired, as shown by a need for the assistance of another person, in at least one activity of daily living or two instrumental activities of daily living housekeeping, shopping, preparing meals, managing money, laundry, using transportation, telephoning and taking medication.• Housekeeping services include assistance with the following tasks on behalf of or to assist a client:– Making and changing beds;– Dusting and vacuuming the rooms which the client uses;– Light cleaning of the kitchen, bedroom and bathroom;• Personal care services include assistance with only the following tasks on behalf or to assist a client:– Assisting with tasks listed in the housekeeping services;– Bathing of client in bed, tub, or shower; (2) The contract period will be for two years. (3) All the questions must be submitted no later than September 30, 2026
