EXTRA-78008 - USA (Ithaca, New York) - Housing Solutions and Intensive Supports Services - Deadline November 17,2026

(1) Vendor needs to provide housing solutions and intensive supports services to the nonprofit organization located in Ithaca, NY. – Provide housing and case management for the implementation of transitional housing and wraparound services programming. – Coordinate rapid access to transitional housing, intensive case management and peer support, and streamlined care for individuals experiencing or at–risk of experiencing homelessness who have substance use with or without co–occurring mental health disorders contributing to frequent use of crisis response services or interactions with law enforcement. • Securing and management of transitional housing units.• Provision of Intensive Case Management (ICM) and peer support for linkage and access to services.• Streamlined coordination of client access to services and care.(2) All the questions must be submitted no later than November 10, 2026. (3) The contract period will be for four years.
