(1) Vendor needs to provide strategic plan development services.– Strategic planning consultant or consulting firm to facilitate the development of a comprehensive data–informed, actionable Strategic Plan. – Strategic Plan shall establish clear organizational priorities, goals, measurable objectives, strategies, performance measures, and implementation actions– Community health data, trends, and identified health disparities, with particular focus on populations experiencing sever health inequities– Existing focus group and community engagement reports– Social determinants of health and demographic and socioeconomic trends– Workforce capacity and organizational development, including recruitment, retention, succession planning, staff training and leadership development– Existing programs, initiatives, partnerships, grants, strategic commitments, and mandated requirements– Multi–tiered internal staff retreats and leadership sessions, ensuring broad staff engagement and consensus–building across all levels of the organization– Performance management and quality improvement, linking strategic objectives directly to performance metrics, Key Performance Indicators (KPI’s) and continuous quality improvement– Stakeholder, staff, leadership, partner, and community input– Fiscal sustainability and resource requirements, distinguishing between actions achievable with existing resources and those requiring additional investment(2) All questions must be submitted no later than October 12, 2026.(3) The contract period will be for one year.
