INSU-6433 - USA (Gilbert, Arizona) - Life/AD&D, Dependent Life, Voluntary Life/AD&D and Critical Illness Services - Deadline October 2,2026

(1) Vendor needs to provide Life/AD&D, dependent life, voluntary Life/AD&D and critical illness services to the government authority located in Gilbert, AZ.– Provide a designated Account Manager and Account Management Team– Provide a designated call and claim unit with toll free telephone access.– Provide customer service to answer inquiries on claims, eligibility, provider network, services, coverage, or other inquiries from participants Monday through Friday from 8:00 AM to 6:00 PM (AZ time)– Provide bilingual communication/educational materials including Policy Booklets/Certificates– Provide Policy Booklets/Certificates in PDF format in time for Open Enrollment (April 2027) Please provide a sample of your policy booklet/certificate in your response.– Provide claims forms– Receive claims and process payments of benefits in accordance with the plan designs for all claims incurred– Correspond with participants if additional information is necessary to complete the processing of claims– Determine benefits payable under the Plan, pursuant to the terms and conditions of the District’s Benefit Plans(2) All questions must be submitted no later than September 14, 2026.(3) The contract period will be for one year.
