(1) Vendor needs to provide partner–based outreach service. • Provide partner–based outreach to individuals and families that may be eligible for health coverage through provide direct enrollment assistance and system navigation to consumers, regardless of health coverage eligibility.• Expand and deepen partner–led outreach efforts to ensure individuals and families are informed about and connected to agency services.• Conducting outreach activities to inform local populations of health coverage options and methods of applying primarily for public coverage through the eligibility system.• Provide application assistance, including completing new applications and renewals, reporting changes to clients' cases, and responding to requests for information• Focusing efforts in geographic areas with high rates of uninsured and underinsured, including rural areas; demographic groups, and language minority populations(2) All the questions must be submitted no later than October 7, 2026
