(1) Vendor needs to provide behavioural health risk monitoring and coordination software.– Software solution that supports structured client monitoring, event and incident reporting, and care coordination across providers. – Configurable workflows to support monitoring activities, including outreach, wellness checks, and provider follow–ups.– Documentation of client contacts and observations.– Assignment and tracking of follow–up actions across responsible entities.– Centralized system for reporting clinical, behavioural, safety, and compliance–related incidents– Ability to capture events from multiple provider agencies– Severity classification, escalation rules, and automated notifications for high–risk situations– Comprehensive audit trail documenting reporting, review, and resolution steps(2) The contract period will be for five years.
