Vendor needs to provide customized caregiver training and relief program services to the government authority located in Jefferson, MO. –  Provide customized in–home caregiver training and caregiver relief to families caring for a person with Alzheimer’s disease or related dementias.• Caregiver training – provide caregiver training:– An in–home assessment of the person with Alzheimer’s disease or related dementia and his/her unmet needs that will be utilized to develop customized training for the caregiver to help meet the identified needs of the individual with Alzheimer’s disease or related dementia and reduce stress on the caregiver.– Care plan development to help meet the unmet needs of the individual with Alzheimer’s disease or related dementias and reduce stress on the caregiver.– Training materials and resources for caregivers to help them modify the daily activities of the individual diagnosed with Alzheimer’s disease or related dementias to promote independence.• Caregiver relief:– Assessment and care coordination – This shall include an initial assessment and reassessment(s) of the individual with Alzheimer’s disease or related dementias and needs of the family.– Provide care options, counseling to aid in planning the future care of their loved ones, assistance in developing an individualized plan to meet the needs of the family, and information and referrals for family support services offered by the contractor’s program and through community organizations.• Assistive technology services to relieve caregivers:– Assessment and coordination – Assessment and Coordination shall include an assessment of the individual with dementia to ensure they are appropriate to be home alone with assistive technology and the caregiver’s comfort level with using technology to monitor the individual when they are home alone.– Maintain an approved list or database of possible devices or services that may be selected to meet the caregiver’s needs.– Once a determination by the contractor is made that the individual is safe to be home alone and the caregiver is comfortable with using assistive technology to monitor them, the contractor shall discuss potential devices and services available.
