(1) Vendor needs to provide intensive case management services to the government authority located in Elmira, NY.– Deliver intensive family or person–centered case management services focused on self–sufficiency, housing stability, and reduced reliance on public assistance.– Program is to reduce or prevent ongoing reliance on public assistance by stabilizing housing, supporting income growth, and addressing barriers through strength‑based, trauma‑informed, and innovative practices.– Program is designed to help households achieve and maintain stability so that public assistance is not needed long‑term.(2) The contract period will be for one year.
