The Berwyn North SD 98 is seeking proposals for a new or a renewed managed Fiber Optic Wide Area Network, or WAN, services to connect its school buildings together at a speed of 10-Gbps to terminate at the headend, Lincoln Middle School, 6432 W. 16th St., Berwyn, IL 60402 as per RFP specifications. Please note, if Cloud-based services, the headend must be a minimum speed of 30-Gbps. Please provide pricing for a 1-year contract. Berwyn North SD 98 currently has a 5-Gbps fiber Internet connection and is seeking proposals for a renewed fiber Internet connection to terminate in the MDF/server room at the Lincoln Middle School, 6432 W. 16th St., Berwyn, IL 60402 for one year. Service providers submitting proposals in response to this FCC Form 470 must be in compliance with rules and orders governed by the Federal Communications Commission. The vendor must agree to comply with rules and regulations of USAC. Vendors must be familiar with billing methods and timelines associated with USAC. Vendor must be in good standing with the USAC and not red-lighted. The vendor must include their SPIN number on the bid/proposal. Service providers responding to this Form 470 should clearly identify E-rate Eligible and Ineligible equipment and pricing. Any ineligible products, services or components bundled with eligible products or services must be listed separately from the eligible products or services. Prices for these ineligible products or services must be allocated out of the price for the eligible services; failure to do so can lead to rejection of the bid. Allocations must comply with USAC rules which specify that the allocation methodology must be based on tangible criteria and reach a realistic result. It is the Service Providers responsibility to access the Portal (EPC) to determine if there are any addenda or changes for this request.
