(1) Vendor needs to provide rural residency program service to the government authority located in Iowa. • Appoint Residency Program Director or identify a Program Director in development.• Develop and maintain an compliant Program Governance Structure, including recognized Sponsoring Institution documentation, bylaws, and faculty appointment processes.• Develop an initial risk management framework that identifies potential barriers that may impede progress toward establishing the rural residency program or a rural track within an existing program.• Develop and submit a validated sustainability pro forma.• Finalize and submit the Graduate Tracking Plan that includes: NPI, practice locations, specialty/subspecialty, and employment status.• Prepare for enrolling the first cohort of residents in the academic year immediately following the grant period.• Implement the operational graduate tracking system, prepared to track all residents once the program’s first cohort graduates.(2) All the questions must be submitted no later than Aug 7, 2026.
