(1) Vendor needs to provide primary accountable care transformation technical solution for case management. – The objective of the procurement is to supplement current state technology infrastructure through case management that will allow for assignment (attribution) and case tracking to support the PACT model. – The platform would enable state staff to oversee care management programs, provide an option for small or remote primary care providers without their own care management tool to participate in the PACT model, and support justice–involved youth targeted case management.– The case management tool is intended to provide care coordinators, case managers, social workers, and clinical teams with the technology infrastructure necessary to manage and track the full scale of member care in a unified, interoperable, and configurable platform for overall project timeline.– This may include maintaining comprehensive longitudinal member records incorporating key clinical, claims/encounter, care coordination, and social determinants of health (SDOH) data.– Improve care coordination and member outcomes with modern case management tools with the ability to document and communicate interventions.– Support documentation aligned to chronic disease and behavioral health integration.– Support evidence–based chronic disease management for rural Medicaid members.(2) All the questions must be submitted no later than September 17, 2026. (3) Letter of Intent to Respond must be submitted no later than September 17, 2026. (4) The contract period will be for two years.
