(1) Vendor needs to provide outpatient remote monitoring (RPM) services to the government authority located in West Virginia.• Reducing avoidable acute care visits and admissions: proactively monitor and manage patients in home and community settings to prevent avoidable emergency department visits and hospitalizations.• Improving Health Outcomes: Demonstrate measurable improvements in the management of chronic conditions for rural residents.• Enhancing Healthcare Access: Bridge the gap in care for rural populations by deploying scalable, cost–effective, and user–friendly solutions.• Fostering a Sustainable Ecosystem: Propose models that can be sustained beyond the initial grant period, potentially through integration with value–based care arrangements and the State's evolving data infrastructure provided by the Connected Care Grid Data Spine and Health Information Network.(2) All question must be submitted no later than June 12, 2026.
