Description
San Mateo County's Human Services Agency (HSA), through its Children and Family Services (CFS) branch, supports vulnerable children and families with services like adoption, foster care, and transitional support. To aid former foster or probation youth (ages 18–25), CFS is seeking proposals from organizations to deliver Transitional Housing Program-Plus (THP-Plus) services. These services offer up to 36 months of supervised housing and supportive services to help youth transition to independent living. The goal of this program is to prevent homelessness among former foster youth; provide age-appropriate housing; support education, employment, and independent living; and foster self-sufficiency and long-term stability. Eligible participants are those who were in out-of-home placement at age 18 and currently reside in San Mateo County or its neighboring counties. The program includes housing options like Single Site, Host Family, and Scattered Site models. Each participant will work with support staff to develop a Transitional Youth Case Plan (TYCP) focused on education, employment, and life skills development. In August 2024, the County awarded an agreement to StarVista to operate the THP-Plus program. However, in August 2025, StarVista terminated the agreement because the organization closed. CFS currently provides THP-Plus services and is seeking a provider to take on program operations/THP-Plus services Propoers must be certified to operate as a THP+ provider. If Proposer is not currently certified, Proposer must diligently work to become certified prior to the start of services. Information about how to become a certified THP+ provider can be found in ACL12-44 and the John Burton Advocates for Youth website. The selected contractor will operate the program as outlined in the RFP. The contract will have an initial term of three years, effective approximately July 1, 2026, with an option to extend it to up to two additional years. All terms of the contract in effect at the time of any extension shall remain in effect for the duration of the extension. The County anticipates this agreement would serve 35 youth annually, though the contract will not guarantee a minimum number of referrals per contract year
