(1) Vendor needs to provide living assistance services to the government authority located in Paterson, NJ.• Provide One lead contractor shall administer the program and provide access to low–barrier sober living placements.• The lead contractor shall receive applications and referrals, verify and document agency residency and financial eligibility, coordinate appropriate available placements, administer assistance, maintain participant–level financial records, and coordinate recovery resources.• The contractor shall confirm placement within three business days after admission and follow up at least weekly while assistance is provided• These contacts may be integrated with peer support services– Linkages must be established with providers offering services such as:• Medication–Assisted Treatment (MAT)• Mental health services• Employment and vocational training• Transportation to essential services• Passaic County Office of Recovery• Broker Agency Provider only: Qualified Sober Living residences(2) The contract period will be for one year.
