(1) Vendor needs to provide fully insured vision benefits services to the government authority located in Phoenix, AZ.• Provide a fully insured Buy–Up Voluntary Vision plan for eligible City Participants and that can provide to the following services• Provide top–tier customer service(s) to the city and its’ plan participants.• Provide consistent, responsive, and professional administrative service supported by verifiable performance guarantee measurements.• Provide clear, effective and reliable communication(s) and assistance that aids participants navigation and understanding of benefit offerings, participant portal, network(s) access, and services.• Provide secure, multi–factor authentication, Participant self–service portal, web site, and mobile app that includes a search feature for up–to–date contracted providers by geographic area, service specialty and option to print ID card.• Provide for Monthly premium billing on a self–billed basis.• A toll–free phone number with 24/7 interactive voice response (IVR).• Website, participant portal, mobile app access to account information benefits, and provider directory for the Participants.• Employer portal with limited access to account and membership information.• Claims administration: accept and accurately process claims timely for all participants(2) All questions must be submitted no later than August 7, 2026.(3) The contract period will be for five years.
