STAFF-16946 - USA (Chicago, Illinois) - Medical Director Service - Deadline October 13,2026

(1) Vendor needs to provide medical director service to the government authority located in Chicago, IL. – The Medical Director shall be an Illinois–licensed physician to provide services, as needed.– Medical Directors are independent contractors and not subcontractors, agents, or employees.– Training will be provided to the Medical Director to facilitate requirements and infection control practices consistent with the Infectious Disease Control Policy– The medical care and treatment records of agency individuals contain confidential information. – The Medical Director is prohibited from making any copy of or otherwise distributing individuals’ records, nor otherwise disclosing any information from these records, except as necessary to perform services required by the contract or unless otherwise legally authorized.– The Medical Director shall be responsible for their own transportation, lodging and healthcare coverage and cost of such shall be included in the all–inclusive hourly rate listed.(2) The contract period will be one year.
