Description
(1) Vendor needs to provide veterinary services to the government authority located in Springfield, MO.• Coordinate veterinary activities in order to maintain the health and wellbeing of all animals at agency• Shall examine and treat all animals, prescribe medications as required, oversee a preventive health care program, coordinate diagnostic and laboratory services, and carry out animal anesthesia, tranquilization, and/or euthanize as needed.• Shall examine, test, and vaccinate as necessary and/or required• Shall prepare any necessary permits for controlled substances used in veterinary program and maintain said controlled substances• Shall make routine farm visits within seventy–two (72) hours of the request being made.• Shall be able to refer to another veterinarian specialist if deemed necessary• The estimate for such work shall be submitted to the Division of Purchases contract administrating Buyer. – Should said estimate be accepted, prior to the Service Provider proceeding with work, a written purchase order to proceed must be issued by the agency• The contract may be terminated for cause. Cause can include, but not limited to repeated incidences of poor workmanship, lack of reasonable communication (24 hours or longer after notification), or gross negligence as determined by agency(2) The contract period will be for one year.
