MEDI-8107 - USA (Phoenix, Arizona) - Independent Medical Evaluation Services - Deadline October 16,2026

(1) Vendor needs to provide independent medical evaluation services to the government authority located in Phoenix, AZ. – Medical record evaluation services: Review an applicant’s medical records before the disability exam to understand the applicant’s medical condition.– Preparing a “Summary of Medical Records“ for use by the examining physicians. – Reviewing the applicant's claimed disability (or disabilities), as described on their disability application, plus all past medical records, obtained from various sources, to determine the medical specialty (or specialties) most applicable to evaluating the applicant's claimed disability (or disabilities).– Scheduling of medical appointments shall only take place when all medical records have been provided to the firm. – Preparing correspondence to provide to the agency’s staff and applicant regarding the scheduled appointments.– Rescheduling missed or cancelled appointments as needed.• Examination services:– The examining physicians are required to verify the identities of applicants when they arrive for appointments.– Complete the appropriate medical questionnaires which will be provided by the agency for each application.• Post medical appointment services:– Medical reports will be prepared for each disability medical evaluation.– Review of records: A description detailing the medical records reviewed for the disability medical evaluation.– Time spent with the applicant: A summary of the time spent with the applicant for purposes of the disability medical evaluation.• Provide signed copies of the disability medical evaluation reports to the Local Boards’ staff responsible for the disability application processing.(2) All the questions must be submitted no later than September 29, 2026. (3) The contract period will be for three years.
