(1) Vendor needs to provide life and disability insurance and employee assistance program broker services to the government authority located in Tulsa, OK.– provide Broker Services for life and disability insurance and a comprehensive employee assistance program for employees.– Prohibited Limitations and Exclusions:– Insurance provider must agree to waive all pre–existing condition limitations, as well as any actively.– Reporting, monitoring, and eligibility administration:– Broker shall, on behalf of the agency, require and confirm that any recommended insurance carrier provides reporting, monitoring, and eligibility tracking and notification services.– Employee and district notification:– The employee when a dependent is approaching loss of eligibility due to the maximum eligible age under the applicable benefit plan.– Reporting requirements:– List of participants including employee and dependent name, date of birth, social security number, coverage status (such as employee, spouse, child) by department, site or employee class or other classifications.(2) All the questions must be submitted no later than September 22, 2026.(3) The contract period will be for one year.
