Public Health Modernization- Community Health Data Platform

Washington County Public Health (WCPH) is seeking a contractor to design and develop a community health data platform. This data platform will serve as a tool to analyze community health trends and plan for program services through data-driven insights and analysis. The collection and analysis of health data informs the identification of health disparities within the community and the factors that contribute to them. WCPH strives to facilitate community action for improvements to address health inequities. Community members and partners are more able to affect change if they are aware of the health status of the community. This data platform will be used to share health findings for the purpose of translating data into action and monitoring the equity impacts of program services.
