EXTRA-76902 - USA (Pomona, New York) - Crisis Respite Bed Service - Deadline October 15,2026

(1) Vendor needs to provide crisis respite bed service to the government authority located in Pomona, NY. – Implement a new residential crisis respite program that will create 6–8 new crisis respite beds in a single home in a central location.– The residence should be staffed 24/7 and, ideally, staffed by peer workers and provide food, board, support services, discharge planning and follow–up. The program should have a 24–hour warmline for referrals.– On–site supervision for a crisis residence is necessary for support and ongoing training of staff. On–site supervision is not required 24/7. Program supervision must be addressed in the staffing plan– Support services should include but are not limited to:• Referrals to new programs/resources and connections with natural supports• Engaging in crisis intervention planning• Assistance in activities of daily living• Re–connection with previous/current service providers• Transportation assistance as appropriate• Benefits assistance• Peer support• Availability of wellness activities• Skill building• Discharge planning• Post–discharge check–ins for up to one month after discharge• Provide a smoke–free indoor environment• Be staffed 24 hours a day, 7 days a week(2) All the questions must be submitted no later than October 6, 2026.(3) Pre–bid meeting will be held on October 1, 2026.
