Description

(1) Vendor needs to provide home safe program services to the government authority located in Pasadena, CA.• Provide short term housing stabilization services, which may include advocacy, resolving barriers, short–term financial assistance, and other support services, to clients of Adult Protective Services.• Staff shall have experience in mitigating housing insecurity, case management, and working with older adults and dependent adults and their specific needs.• Contractor shall provide housing navigation and case management services with the goal of stabilizing the client’s housing situation.• Contractor shall provide as–needed short–term financial assistance to avert housing instability.• Contractor shall follow progressive engagement model principles, which include err on screening in, not out; complete “warm“ handoffs when referring to other programs; and, at case closure, providing every client with a plan to increase housing stability and knowledge of who to call in a future housing crisis.(2) The contract period will be for one year.(3) All the questions must be submitted no later than September 17, 2026.
