(1) Vendor needs to provide financial management services to the government authority located in Minnesota.– To support the administration of self–directed caregiver grants for at–risk individuals, caregiving for an older adult, through the use of a Fiscal Management Services.– Create, operate and manage a statewide self–directed program to provide support to family, friend and neighbour caregivers of older adults.– The purpose of the support is to allow caregivers to sustain their roles and support the people they are caregiving for to remain at home. – Caregiver consultants are responsible for initial and ongoing assessment with caregivers to determine the caregiver’s needs and make appropriate referrals, including to the self–directed grants program. – The Responder will be responsible for determining eligibility and administering self–directed program with Participating Caregivers.(2) All the questions must be submitted no later than September 15, 2026.
