(1) Vendor needs to provide home personal care services to the government authority located in Lyons, NY.• Need for the assistance of another person, in at least one activity of daily living (bathing, dressing, toileting, being continent of urine and/or feces, transferring, walking and eating) OR two instrumental activities of daily living (housekeeping, shopping, preparing meals, managing money, laundry, using transportation, telephoning and taking medication).• Services are to make it possible for these frail or disabled seniors to be able to live independently at home as long as possible.• Assisting with tasks listed in the housekeeping services;• Assisting with self–administration of medication; • Performing routine skin care, including applying non–prescription skin care products.(2) All questions must be submitted no later than July 31, 2026.
