(1) Vendor needs to provide pharmacy consultant services to the government authority located in Phoenix, AZ.– Assist as needed with the coordination of pharmaceutical services if and where multiple services providers are used.– Assist as needed to develop intravenous therapy procedures.– Identify any drug therapy irregularities such as:– Use of medication without supporting diagnosis.– Use of inappropriate medication dose.– Concurrent use of medication that could potentially interact.– Duplicate therapy.– Continual use of medication that is no longer required.– Presence of medication–related adverse reaction.
