EXTRA-73896 - USA (Des Moines, Iowa) - Colorectal Cancer Screening Evidence-Based Interventions Service - Deadline August 31,2026

(1) Vendor needs to provide colorectal cancer screening evidence–based interventions service to the government authority located in Des Moines, IA.     – Provide implementation of evidence–based interventions (EBI) to promote and support colorectal cancer (CRC) screening.    – Address patient barriers to screening by connecting individuals to resources and enhancing clinical workflows through electronic health records (EHR) will also be a focus of this grant.– Program successes, expand partnerships, enhance quality improvement (QI) as well as increased technical assistance, training and awareness opportunities for contractors and the community.– Provide guidance and resources to support stool–based testing (FIT) and colonoscopy services for eligible patients after award.– Provider education focuses on giving clinicians the knowledge, tools, and up‑to‑date guidance needed to recommend and deliver screening effectively.– Implement evidence–based interventions to increase CRC screening.– Enhance electronic health record systems.(2) All the questions must be submitted no later than August 06, 2026.(3) The contract period will be for six years.
