(1) Vendor needs to provide pre–employment background investigation services to the government authority located in Salem, OR. – Provide background investigation services related to new and current employees, contractors and volunteers. – Investigations shall include phone and in–person interviews and databases or reports.I. Investigation report searches: • Social Security Number (SSN) trace, verification and validation: Conduct a search of credit headers and other database sources to provide history of the applicant’s names and addresses as they originate from credit headers, however, credit history itself will not be considered by agency.• Criminal record searches: The primary source search must be performed in a multi–state/multi–jurisdictional criminal records locator and other similar commercial nationwide database with validation.• National sex offender registry check: Reported search information may include: type of offenses, known aliases, date of birth, addresses, and any violations of registration requirements. • Driving records (motor vehicle records) search: Reported search information include license issue date, license expiration date, violations, disciplinary actions, convictions, revocations, suspensions, accidents, status and restrictions.II. Investigation report submission:• Submit background investigation reports within 4 days of completion via mail or hand delivery, reports shall consist:– Physical description– Driving record– Criminal record– Military history– Family information– Educational history(2) All the questions must be submitted no later than September 16, 2026. (3) The contract period will be for three years.
