CASE-1806 - USA (Buffalo, New York) - Case Management and Public Information and Education Services for Older Adults - Deadline October 20,2026

(1) Vendor needs to provide case management and public information and education services for older adults to the government authority located in Buffalo, NY. • Case management – Conduct assessment and reassessments of eligible clients – Person–Centered care planning aligned with client wishes and values – Service arrangement and coordination – Responsible for having an assigned caseload – Documentation in Department specific software application – Ongoing follow–up and monitoring with clients – Develop goal–oriented care plans – Referrals to both county funded and non–county funded services – Internal service monitoring • Public information and education – Develop an annual Public Information & Education work plan for approval by the Department – Plan and conduct educational presentations, workshops, and outreach events throughout Erie County – Work with the Department to track, report, and measure outcomes of work performed – Coordinate programming with senior centers, libraries, municipalities, faith–based organizations, housing sites, healthcare providers, and community organizations – Recruit and coordinate qualified presenters and subject matter experts – Develop, update, and distribute educational materials in print and electronic formats – Promote educational activities through print media, social media, local media outlets, community partners, and other communication channels – Staff outreach tables at community events as requested by the Department – Coordinate event registration and participant follow–up(2) A non–mandatory pre–bid meeting will be held on October 1, 2026.(3) All questions must be submitted no later than October 2, 2026.(4) The contract period will be one year.
