(1) Vendor needs to provide forensic auditing and investigative services to the government authority located in Eagle Lake, FL.– The nature and amount of the reporting deficiencies or errors that resulted in the repayment. – The individuals involved in preparing, reviewing, approving, certifying, or submitting the applicable Treasury reports and their respective responsibilities. – The circumstances and causes that resulted in the inaccurate or unsupported reporting, including whether the matter resulted from misunderstanding of program requirements, inadequate documentation, insufficient review or internal controls, negligence, intentional action, or other identifiable causes. – If the initial review identifies transactions or patterns indicating potential fraud, intentional misconduct, procurement violations, material financial loss, or other significant irregularities, the firm should notify the City and obtain authorization before materially expanding the investigation.
