(1) Vendor needs to provide incident management, audit management, and risk assessment software.• Incident Management – a fully customizable incident reporting, routing, investigation, and analytics solution covering client safety, complaints and appeals, environment of care, staff misconduct, abuse/neglect/exploitation reporting, cybersecurity, staff injury, infection control, medication errors, and corporate compliance reports.• Audit Management – a centralized solution to organize all third–party audit requests, processes, deliverables, and corrective action plans, including secure, limited–access collaboration with external auditors.• Risk Assessment and Response – a customizable risk assessment tool with automated inherent and residual risk scoring and an associated task and project tracker for risk response.– Integral Care will:• Designate a project lead and system administrator(s) for implementation• Provide access to legacy system data for migration• Provide necessary technical contacts for SSO and API integrations• Automatic and manual generation of notifications to staff members’ Microsoft Outlook accounts for task assignments, reminders, and requested follow–up• Hosting, storage, processing, and backup of all data within the geographic boundaries of the States.– Risk Assessment and Response• Ability for multiple users to add and assess potential risk areas• Structured fields with predetermined response options for risk rating questions• Automated calculation of inherent risk score based on responses to risk rating questions• Narrative section for staff to identify existing controls– Audit ManagementA customizable solution that provides a centralized location to organize all third–party audit requests, processes, and deliverables, including:• Data entry interface for each audit to track start dates, parties involved, status, requirement of corrective action plans, corrective action plan due dates, close–out date, etc.• Dashboard or reporting mechanism that aggregates and reflects the above information• Data fields that are customizable by an internal administrator–level user(2) All question must be submitted no later than December 7, 2026.
