(1) Vendor needs to provide substance abuse treatment services to the government authority located in Waterford, MI.– Substance abuse treatment services will also be assessed and reviewed for possible use in non–specialty court evaluations or other court related programs.– Treatment should promote wellness as well as symptom–management, address the social determinants of health and mental health, and empower individuals to maintain recovery and achieve successful community tenure.– Able to treat individuals with challenges stemming from substance use disorders, severe mental illness (SMI), justice involvement, homelessness, unemployment and lack of education and training.– Provides an array of treatment options to meet the unique needs of each participant, including group and individual counseling.– Facilitate referrals and provide any care and coordination and level of care change needs for all primary addiction, mental health, and medical conditions. – Develop an individualized treatment plan for each participant.– Nursing monitoring.– Family therapy, if indicated by the individual’s treatment plan.– Educational or instructional groups, once per month.– Coordination and facilitation of mental health treatment.– Discharge planning and case management.(2) All questions must be submitted no later than July 6,2026.
