HEALTH-10440 - USA (Oregon) - Rural Health Access and Social Supports Service - Deadline August 7,2026

(1) Vendor needs to provide rural health access and social supports service to the government authority located in OR.– Priority Area 1: Expand Mobile Healthcare Services– Projects funded under this priority area should expand existing mobile healthcare services to improve access for rural Washington County communities. Examples may include, but are not limited to:• Primary care services• Behavioral health services• Oral health services• Maternal health services• Optometry healthcare services– Priority Area 2: Increase Connections to Social Supports– Projects funded under this priority area should improve coordination and connections to social supports and community resources for rural agency residents. Social supports may include, but are not limited to:• Food and nutrition resources• Transportation support• Housing–related services• Community–based supports that help individuals maintain health and wellbeing(2) The contract period will be for one year.(3) All the questions must be submitted no later than July 31, 2026
