(1) Vendor needs to provide client advocacy services to the government authority located in Santa Ana, CA.– Contractor shall complete a screening and assessment of all referrals received within 30 calendar days to determine whether a referral meets social security administration’s eligibility requirements to receive SSI, SSP, RSDI and SSDI benefits.– Sensitive to literacy, language and socio–cultural issues that may impact the clients.– Integrated, coordinated and easily accessible resources for clients.– Community–based, integrated services that coordinate federal, State and community funding opportunities.– Communicate with designated county liaison, as required by county, to receive client contact information.– Conduct face–to–face, telephone and virtual interviews with clients, parents or social services agency representatives.– Assist clients with the social security administration appeals process through final hearing.– Monitor the case status through the social security administration process and communicate status change to county.– Create and maintain an electronic or physical folder for each client in a format approved by county.– Administer client satisfaction surveys during the final phases of service delivery, prior to the conclusion of services and share results of surveys with county via email on a semi–annual basis.(2) All questions must be submitted no later than October 21, 2026.(3) The contract period will be for three years.
