(1) Vendor needs to provide pre–employment physical examinations service to the government authority located in Anchorage, AK. – Perform physical examinations on an “as needed” basis.– All physical examinations and evaluations must include:• Patient contact information forms.• HIPPA privacy notices as required under applicable law.• Consent form for examination or evaluation.• Release of information form.• Physical fitness opinion form, signed by the Contractor’s medical provider.– All physical examination reports and results must be signed off by agency licensed physician.(2) All the questions must be submitted no later than September 2, 2026.
