(1) Vendor needs to provide medical physicals, audiogram testing and asbestos physical services to the government authority located in St. Joseph, MO.– Annual medical physical exam services for firefighters must include:• Audiogram• Complete Blood Count (CBC) without differential• Chest C–Ray• Electrocardiogram (EKG)• Hepatitis B Surface Antibody Quantitative• Vision Acuity (Titmus)• Lipid Panel• Medical Questionnaire• Tetanus (TDAP) Vaccine• Hepatitis A Antibody– Provide audiogram tests and asbestos physicals for specialized trades workers.– Perform audiogram tests and asbestos physicals annually for specialized trades workers.(2) The contract period will be for two years.
