HEALTH-10656 - USA (Baltimore, Maryland) - Young Adults with Sickle Cell Disease Support Services - Deadline September 16,2026

(1) Vendor needs to provide young adults with sickle cell disease support services to the government authority located in Baltimore, MD.– Provide support for young adults with sickle cell disease that are entering adult sickle cell care.– Develop or enhance an existing comprehensive transition plan.– Program design shall include the development or adoption of evidence informed evaluation tools and action plan templates.– Program design shall include the development of protocols and workflows for administrative components including the development of and feedback on interventional strategies.– Program design will provide services in a health literate manner and will focus on family functioning, including supporting positive relationships and communication emphasizing independence and self management.– Providing services to assist with positive provider–patient relationships and communication.– Program design shall include systems for referral, including coordination with primary care providers, mental health professionals.– Strengthen positive relationships among patients, parents, and providers.– Integrate a transition coordinator skilled in assessing psychosocial needs, navigating complex medical–legal systems, executing crisis interventions.(2) All questions must be submitted no later than September 9, 2026.(3) The contract period will be for one year.
