MEDI-8152 - USA (San Leandro, California) - Hospital-Based Violence Intervention Program Services - Deadline November 9,2026

(1) Vendor needs to provide hospital–based violence intervention program services to the government authority located in San Leandro, CA.– Comprehensive evidence–informed hospital–based violence intervention program and related services.– Providing bedside engagement and healing–cantered case management services.– Annually provide hospital–based engagement and intervention services for up to 50 individuals, age 34 and under, in the emergency department or at the hospital bedside for victims of violence. – Deliver tailored case management services, including mentoring, life skills development, wrap–around care, and trauma–informed services, to support victims and reduce the likelihood of further violence to all participants who indicate interest. – Administer violence prevention/intervention activities such as violence interruption, street outreach, community education trainings, or group therapy sessions. – Provide follow–up services through linkages to community–based resources, mentoring, home visits, and other ongoing care and support services. – Address social determinants of health on a case–by–case basis to support program–enrolled clients in making lifestyle changes.
