CASE-1782 - USA (Martinez, California) - Assertive Community Treatment and Full Partnership Intensive Case Management Services - Deadline August 28,2026

(1) Vendor needs to provide assertive community treatment and full partnership intensive case management services to the government authority located in Martinez, CA. – Provide Assertive Community Treatment (ACT) and Full–Service Partnership (FSP) Intensive Case Management (ICM) for eligible Medi–Cal Transitional Age Youth countywide under the adult redesign.– Targeted Case Management (TCM): A service that assists a beneficiary in accessing needed medical, educational, social, prevocational, vocational, rehabilitative, or other community services.–  Peer Support Services: Culturally competent individual and group services that promote recovery, resiliency, engagement, socialization, self–sufficiency, self–advocacy, development of natural supports, and identification of strengths.– Crisis Intervention Services: Crisis Intervention services last less than 24 hours and are for, or on behalf of, a beneficiary for a condition that requires more timely response than a regularly scheduled visit.– Employment and Education Support Services: Services that support recovery by assisting members in managing their mental health conditions in vocational or educational settings.– Medication Support Services: Services include prescribing, administering, dispensing and monitoring of psychiatric medications or biologicals that are necessary to alleviate the symptoms of mental illness or treat SUDs.(2) All the questions must be submitted no later than August 12, 2026.
