RFP No. ACH-900726 Family Resource Centers - Lead Agencies

It is the intent of these specifications, terms and conditions to describe the school-linked health and wellness services being requested by Alameda County Health (AC Health) to reduce health disparities and maximize positive health outcomes for families in Oakland, San Leandro and Hayward. The awarded bidder(s) will join AC Health’s Family Resource Centers (FRC) Initiative to design and provide school-linked health and wellness services in Oakland, San Leandro and Hayward school districts to address inequities of access to health services, public benefits and culturally appropriate parenting resources. As FRC Lead Agencies, they will work closely and collaboratively with host school districts to provide services that align with school district priorities and County initiatives. The County intends to award two 1-year contracts (one for the Mid-County FRCs and one for the Oakland Central FRC) for the approximate term of August 1, 2026 – June 30, 2027 (with the option to renew annually for 4 additional years) to the Bidder(s) selected as the most responsible Bidder(s), whose response conforms to the RFP and meets the County’s requirements. Table 1 below summarizes the two award regions, school districts each lead agency will work in, and anticipated award amounts for the initial contract. Oakland Central Family Resource Center (FRC) - Lead Agency Oakland Unified School District (OUSD) $123,448 Anticipated Total Funding $347,314 This RFP includes two award regions. A Bidder may submit a bid for one or both of the award regions. Bid responses must clearly state if the Bidder is bidding for (i) the Mid-County FRCs award only; (ii) the Oakland Central FRC award only, or (iii) both. If bidding for both awards, Bidder must follow the detailed instructions included in Exhibit A—Bid Response Packet, Required Documentation and Submittals below, and include separate, clearly labeled submittals where required.
