(1) Vendor needs to provide HIV hospice care and mental health services to the government authority located in San Francisco, CA. – HIV hospice care and mental health services are for HIV–positive residents.– Hospice care requirements provide nursing care and supportive health services to maintain daily living and improve end–stage quality of life for residents with the goal of reducing unnecessaryhospitalizations.– Professional Resident Care Intake Residents by providing assessment of psychiatric, substance use, behavioural health, medication, and palliative needs.– Paraprofessional Resident Care Provide 24–hour attendant care with the appropriate supervisory and support staff to effectively manage these services.– Mental Health Services Provide assessments, develop Individualized Service Plans (ISP) in partnership with Residents, tailor interventions to Residents, complete treatment plans, and complete safeand appropriate discharge plans when needed.– Mental Health Services Objectives a licensed mental health professional as having severe impairment related to substance use and mental health history will be assessed for behavioural health needs and referred to Services(2) Pre–bid meeting will be held on September 9,2026.(3) The contract period will be for five years. (4) All the questions must be submitted no later than September 16, 2026
